The computer could
just need to be restarted

If it's an external
modem then it may just need to be turned off and back on.

The init string going
to the modem could be bad and the modem is responding back ERROR
instead of the OK the computer is expecting.

Fax or voice mail
software could be turned on and is not letting other programs use
the modem.

If it's an external
modem then the cable could be unplugged, bad or the modem could be
off or have it's power supply unplugged.

The modem driver could
be installed wrong and need to be reinstalled.

The setting for which
port the modem is on could be wrong.

On old Non-Plug &
Play Modems, the modem could have a conflicting IRQ or COM port set.

The modem could be
physically broken.

There isn't actually a
modem present on the computer!
